改良雙環法乳暈縮小整形術

[摘要]目的：探討改良雙環法乳暈縮小整形手術的方法及其臨床應用價值。方法：在乳頭根部及乳暈外緣內約1.5cm處設計雙環形切口，去除雙環之間乳暈皮膚，然後設計2～4處楔形切口，切除乳暈多餘的皮膚後減張對位縫合。結果：2008年8月-2015年3月，共有32例女性患者接受了改良的雙環法雙側乳暈縮小手術，術後效果良好。隨訪4～12個月，乳暈直徑較術後無明顯增大，切口瘢痕不明顯，無明顯皮膚皺褶，乳暈及乳房比例適當，外觀形態滿意。結論：改良雙環形結合楔形切除的乳暈縮小整形術式適合不同程度的乳暈肥大患者，避免了明顯的瘢痕及乳暈術後繼發畸形，是一種美容效果良好的乳暈縮小整形方法。

Abstract： Objective To explore the method of modified round block technique areola reduction and its value of clinical application. Methods Design double circular incision at the base of the nipple and about 1.5cm to the outer edge inside the areola individually，after the areola skin resection，2 to 4 wedge-shaped incision was designed in areola area in order to excise the areola skin redundant， and the skin sutured with tension reduced. Results Between August of 2008 and March of 2015，a total of 32 cases of female patients accepted the modified round block technique for bilateral areola reduction surgery，postoperative appearance is good.Following up of 4 to 12 months，the patients without obvious areola diameter increased，also without visible scar and _disibledevent=1.5cm），選擇2～4個楔形切口就能縮短3.5～6.9cm，從而縮小與內環的差距，減少術後縫合時的皺褶。更多楔形切口不僅會造成手術時間延長，而且可能影響皮瓣血運，因此更大的周徑可通過適當增大楔形切口的角度來實現，不過過大的角度也會增加縫合時的張力以及增大術後乳暈的直徑，因此筆者推薦采用2～4個角度為30°～60°的楔形切口較為適宜。
