成年女性雙頜前突患者種植支抗矯治後鼻唇頦協調性分析

[摘要]目的：通過種植支抗矯治成年女性雙頜前突患者，分析矯治後患者鼻唇頦協調性。評價種植支抗矯治成年女性雙頜前突患者的治療效果。方法：選取使用拔牙後種植體支抗矯治雙頜前突成年女性患者15 例，對治療前後的頭顱側位X線片進行測量分析，通過分析治療前後軟硬組織的變化，分析研究矯治後患者鼻唇頦協調性。結果：矯治後患者鼻唇角增大，上下唇厚度增加，相對於E線的突距減小，頦溝傾角和頦厚在治療後明顯增大和增厚，頦唇溝深度加深，與治療前相比較，差異具有統計學意義（P

Abstract： Objective To explore the harmony of nose-lip-chin relationship and to examine the effect in adult female patients of bimaxillary protrusion after orthodontic treatment with micro-implant anchorage. Methods 15 cases with bimaxillary protrusion，were treated with 4 first premolar extractions'followed by anterior retraction with micro-implant anchorage.The cephalometric films before and after treatment were measured and analyzed to explore the harmony of nose-lip-chin relationship. Results The findings indicated that NLA，UL-U1，LL-L1，Pg’B’-FH，Pog-Pog'and mentolabial sulcus depth increased，UL-E and LL-E decreased.The effect demonstrated significant differences between pre-and post-treatment.but the relationship between the maxillary and mandible showed no significant changes.Patients'appearances and the harmony of nose-lip-chin relationship were all significantly improved after the treatment. Conclusion Micro-implant might improve the facial profile and the harmony of nose-lip-chin relationship in adult female patients of bimaxillary protrusion，.but the relationship between the maxillary and mandible showed no significant changes.

雙頜前突是廣東地區口腔正畸門診常見的一種錯牙合畸形，雙頜前突的主要特征：上下前牙前突，即口唇部突出或外翻，開唇露齒，頦部相對後縮，頦肌緊張；鼻唇頦關係不協調。對於這些情況患者要求急需改善審美的缺陷，正畸和正頜治療均可以提升麵部的協調性，正畸治療可以矯正牙槽前突通過直立和內收前牙。而正頜治療通過改變部分頜骨的位置減少前突，兩種治療方法均可以減少麵突度改善側貌。近年來，種植支抗的應用，解決了部分隻有正頜才能治療的問題，傳統正畸治療比較關注是否達到中性咬合關係這個目標，在現代隨著觀念的轉變，是否能獲得協調美觀的軟組織側貌同樣重要。本研究旨在通過治療探討種植支抗下如何提高改善雙頜前突成年女性患者的鼻唇頦協調性。
1 資料和方法
1.1 一般資料
選取2010年3月～2012年12月深圳市第五人民醫院口腔醫學中心就診的15例成人女性安氏I 類雙頜前突患者為研究對象，年齡19～32歲，平均22.5歲。患者納入標準：上牙列輕度或無擁擠，上前牙無先天缺牙，上前牙唇傾，患者側麵貌為凸麵型；矯治為拔除雙側上下頜第一雙尖牙；MBT直絲弓矯治，種植支抗聯合滑動法關閉拔牙間隙。患者治療前後拍攝X線頭顱側位片，記錄數據。
