ThedangerofinvokingtheHIV/AIDSepidemictopromotemeasurestoprotectsexualminorities(especiallygay,bisexualandtranssexualpersons)isobvious.Linkingalreadystigmatizedminoritiestoanepidemicthatisthesourceofignoranceandfear,mayonlyaddtotheburdenofstigma.Nevertheless,theexistenceofthiscohortofpeopleatriskdemandsinitiativesspecificallyaddressedtotheirrisk.Suchinitiativeswill,totheextentthatitisrelevantandpossibleinChina,drawupontheexperienceofothercountries.Itwillrequirelegalinitiatives,affordinglegalprotectionsthataccepttheHIV/AIDSparadox.
Inthecountrieswhereinitiativeshavebeensuccessful,theyhaverelevantlymobilisedgayrepresentativegroupstospreadknowledgeaboutHIVandthemodesofitstransmission;providedinformationcampaignstargetedatsuchgroups;educatedpoliceandotherofficialssoastoreducetheharassmentandalienationofthiscohort;promotedandadoptedlawsandpoliciesaffordingprotectionandredressagainstdiscrimination,inequalityoftreatmentandviolence;andacceptedstrategiestoencourageamorerationalandinformedpublicawarenessoftheexistenceofsexualminoritiesandanunderstandingoftheirhumanneeds,whicharesubstantiallythesameasthoseofothercitizens.
Mixedmessages:DespitethedangersofmixingthemessagesofsexualequalityandHIVpreventionandcare,theadventofHIVdoesrequireofallsocietiesintheworldamorerationalandinformedapproachtowardstherealworldofhumansexualdiversity.
Thismaybeuncomfortablefortraditionalists.Itwillbeopposedbysomereligionists,inthenameofdivinerevelation.However,certainlyinthecurrentstateofvaccineandtherapeuticdevelopmentsandavailability,nootherstrategyislikelyinpracticetoreducethespreadofHIVandlessentheenormousburdenoftheepidemic.Thatburdenisindividualandsocial.Itiseconomicandthreatenstorobmanydevelopingcountriesofthefruitsofhard-earnedeconomicprogress.Importantchoiceshavetobemadebypoliticalleaders.Somuchiscoldreality.Itistheroleofuniversitiestoprovidethoseleaderswithscientificdataontheextentoftheepidemic;onitspotentialcosts;andonthecrucialdecisionsthathavetobemadetocontainitandreduceitsimpact:personal,social,nationalandglobal.
