[摘要] 目的 檢測急性顱腦損傷患者血清中C反應蛋白(CRP)及乳酸脫氫酶的表達量,探討二者在顱腦損傷中的臨床意義。方法 收集121例急性顱腦損傷患者和50例正常成人的臨床資料,檢測血清中CRP及乳酸脫氫酶的含量,分析其在不同臨床特征及預後表達的關係。結果 急性顱腦損傷患者血清中CRP及乳酸脫氫酶含量升高,重型顱腦損傷中的表達明顯高於非重型顱腦損傷患者,死亡組患者的表達明顯高於存活組。結論 急性顱腦損傷時血清中CRP及乳酸脫氫酶含量升高,在顱腦損傷急性期聯合檢測血清中二者的含量對判斷病情程度及預後有重要價值。

Expression of C-reactive Protein and Lactate Dehydrogenase in Acute Traumatic Brain Injury and Its Clinical Significance

[Abstract] ObjectiveTo detect the expression levels of serum C-reactive protein(CRP)and lactate dehydrogenase in acute traumatic brain injury and to explore the clinical significance of both in the injury. MethodsThe experiment included 121 patients with acute traumatic brain injury as experimental group and 50 adults as control group. The serum CRP and lactate dehydrogenase levels were detected in the two groups,and their clinical significance and relationship were analyzed. CRP是環狀五球體蛋白,由炎性淋巴因子刺激肝髒和上皮細胞合成,可被細胞因子誘導而發揮生物學作用[2,3];在組織損傷等刺激下,被激活的單核細胞釋放白細胞介素-1,刺激肝細胞加速合成CRP,進一步激活巨噬細胞和補體係統,發揮其清除異物及壞死組織的功能。CRP作為急性時相蛋白中較為重要的一種,其升高是組織受損時的保護反應[4]。有研究認為CRP與急性腦損傷及腦血管事件有關[5]。乳酸脫氫酶大量存在於神經元細胞的細胞漿和線粒體中,當顱腦損傷時引起腦組織損傷和腦細胞缺血缺氧,它釋放到細胞間隙再擴散入腦脊液,可通過受損的血-腦屏障進入血液[6],引起血液中乳酸脫氫酶含量升高。Korfias等[7]認為血清中乳酸脫氫酶及不同細胞因子與腦損傷密切相關。本實驗結果顯示在急性顱腦損傷組中CRP及乳酸脫氫酶的含量均明顯高於正常對照組;在中、重型急性顱腦損傷組中的含量明顯高於輕型患者,在死亡組患者血清中的含量明顯高於存活組,提示CRP及乳酸脫氫酶在損傷時表達增高,損傷越重,表達量越高。實驗也提示顱腦損傷早期對CRP及乳酸脫氫酶進行檢測,對判斷顱腦損傷的預後可能有重要價值。急性顱腦損傷可引起強烈的應激反應,使CRP表達增高[8]。急性顱腦損傷後,損傷或變性的神經元中大量乳酸脫氫酶釋放到細胞外,引起血清中含量增加,而且損傷程度越重,神經元變性壞死越多,血清中含量越高。本實驗顯示CRP及乳酸脫氫酶在顱腦損傷中具有正相關性,提示二者在損傷時可能具有協同作用,其作用機製尚需進一步研究。
